Forms for all appointments are usually attached to the appointment email. If you did not receive a document then we may already have your required documentation on file. Your assigned therapist will review your account and make sure all needed documentation has been received prior to your appointment. Check your spam email for messages. The following forms below are in the event the link on the site has not worked properly or if the therapist has specifically requested for you to complete such form. About the Practice

Founded in 2006, Kimberly L. Hillery & Associates, PLLC is a Professional Limited Liability Company established under the laws of the State of Michigan. The practice was created to provide a multi-systemic approach to therapy, recognizing the complex interplay of personal, relational, and community systems in emotional well-being.
